The show's title references The Ugly Duckling fairy tale, positioning makeover as metamorphosis. Weekly episodes feature two women competing for a spot in the season finale pageant, where one is crowned The Swan. Board-certified plastic surgeons Terry Dubrow and Randal Haworth led the surgical interventions. Two seasons aired in 2004, but Fox cancelled the show in early 2005 as ratings declined, preventing a planned third season.
